Hip pain: Hip replacement and Iliotibial Band Syndrome?


Hip replacement and Iliotibial Band Syndrome?I had a hip replacement four years ago. For these four years, I've had a severe pain on my lower thigh, on the outside of the leg near, but not on the knee which causes me to limp significantly. My surgeon diagnosed me as having trochanteric bursitis due to the rubbing of the bursa at the hip replacement site. I'm beginning to have a new theory. Iliotibial Band Syndrome - since the pain is near the knee. Could it be that my leg is now longer or shorter since the surgery and irritating this? Does anyone have more information for me?

"Many patients with osteoarthritis of the hips have one leg shorter than the other, although they may not notice it. At the time of hip replacements surgery, the surgeon always tries to balance the hip and restore leg lengths, if at all possible. In the vast majority of cases, leg length can be restored to within 1 cm of difference (a difference patient cannot notice). Unfortunately on rare occasions, the leg length discrepancy may be greater than this despite using calipers (rulers) during the operation."

As for whether the surgery causes ITBS you would need to see your orthopaedic surgeon to find out.

A problem can occur at the hip called greater trochanteric bursitis. During flexion and extension of the knee the iliotibial band rubs over the femoral condyle which leads to irritation. Weak hip abductors, especially the gluteus medius is often found.

Some other factors that may contribute to this syndrome include genu varum (bow legs), pronation of the foot (subtalar joint pronation), leg length discrepancy.
So it is a possibility.
