Nagging Groin/Testicle Pain?


Nagging Groin/Testicle Pain?For the past three years I have had mild pain in my right lower groin area. Only on the right side and never extreme pain, I would rate the discomfort at maybe a 2 or 3 on a scale of 10 at the worst.

I assumed I had a hernia and was "living with it" until it got worse. Roughly a year ago I decided it was time to get it fixed so I went to a prompt-care to get a referal. The doctor there didn't feel a hernia so she ordered an ultrasound on my lower abdomen. The ultra sound showed nothing. I was refered to a urologist who said he might have felt a light hernia and that he thought everything else seemed fine. He refered me to a general surgeon. The surgeon examined me and I kind of got the feeling he didn't detect a hernia, he recommended that I wait until it got worse. This was a year ago. Roughly a month ago I decided I had enough, so I returned and he examined me and set an appointment for a right inguinal surgery.

I still was not certain that I had a hernia so I set an appointment with another surgeon for a second opinion. He told me that he felt no hernia and thought I might have a problem with my right testicle, possibly a variocele.

The problem with this is that I have read 99% of varioceles occur on the left side and I was recently examined by a urologist and made no mention of it.

Sorry for the novel but here are a few other symptoms:

The pain seems to be most prevelant while sitting or laying... not sure if my mind is on other things when I am up and about and that is why I don't notice it, but I am pretty sure it is worse while driving or resting. This apparently points towards something other than a hernia.

The pain is most prevelant above the right testicle but it has a tendancy to radiate further up the groin towards my hip.

The pain is more of a pulling or dragging sensation as opposed to a sharp pain.

One other monkey wrench to throw in, I have had a history of kidney stones and have had a lythotripsy operation and an old fashion "snatch" operation. The "pain" I am feeling is nothing like the pain I experienced when I couldn't pass the stones.

I am 27 years old and otherwise very healthy.

Anyone have the same problem?

Sounds like a form of orchialgia, basically chronic testicular pain associated with vasectomies. But not limited to those either.

Have you had problems with ejaculating or orgasm? If so that is something to mention to the doctor.

Basically varioceles are the testicle version of varicose veins common on guy's legs. If it is not a hernia, then a variocele is probably the next best answer. About 2% can occur on the right testicle. So it is rare, but not impossible. I do not think kiddney stones should have anything to do with this pain. Its possible, but not by anything I'm aware of.

A couple other possibilities include testicular vasocongestion. This is basically the technical name for blue balls. Its where blood prevents the release of semen and causes an achy pain in one or both testicals. This is usually resolved through ejaculation. However in rare cases, minor surgery is needed to correct the problem.

Vasocongestion can in rare cases also lead to orchialgia which can require surgery to correct. It is also possible that you have an infected lymph node in the area that has become swollen. It is important that they check for any infections before surgery because if they do the surgery and some how rupture the lymph node or expose your body to the infection you can become septic (the infection gets in your blood), which can be fatal.

If ejaculated doesn't resolve the problem or makes it worse then I think you need to find a doctor who is willing to look into alternate possibilities. Hopefully I gave you some things to look into.

You might look into Chronic Pelvic Pain Syndrome. It's basically a condition where muscle problems in the pelvic floor refer their pain to other areas like testicles, penis or rectum. I have it. My uro understands it (rare for uro's) and says she thinks it might be related to those kidney stone removals.

Sitting does make it worse. There's a book called "Headache in the Pelvis" from Anderson at Stanford. You would have mild case.

Absolutely do NOT go to a surgeon for a solution to an unknown problem. You did good by getting a second opinion.
