Two major RISK FACTORS for AVN are: heavy use of corticosteroids over a long period of time and heavy drinking. There are several other less common risk factors. Scroll through this link to find them.

The specific CAUSES of AVN are: joint injury, narrowed vessels, pressure inside the bone. These describe WHAT happens, not WHY it happens:

Hip replacement is an effective treatment for AVN and should have him back on his feet and pain-free. The condition is one of the common reasons that people as young as your fiance need hip replacement (other reasons include injury/fracture, dysplasia). I've had both hips replaced because I suffered from severe osteoarthritis, the most common reason for old people (like me) to need hip replacement.

As for the drinking, I'll leave it up to you to judge whether the drinking is a problem - AVN or no AVN.
