15 YEAR OLD: HIP PAIN GOING ON FIVE MONTHS?I am fifteen years old; My hip pain all started in March when i was running a warm up lap. It all of a sudden started hurting. After this I high jumped in an indoor track meet. It didn't seem to bother me until my jumps at 4'10(which i did clear) and then 5'. It wasn't even that it was hurting terribly it was just at that point I could tell it was holding me back. After this I go back to track practice and visit our trainer. When I went to see him he helped me stretch and did a little stability exercises. He then was watching me walk and then looked at my legs while i was sitting. I have a leg length discrepancy. I believe I saw him for about 2 weeks after practices everyday and finally he referred me to a Sports Medicine doctor.
My pain was tollerable but when I ran it was very hard to tolerate. I went to the doctor and he looked at my legs and said that it wasnt anything too serious. He then said we will give you 6(or so) weeks of Physical Therapy. I thought this was suitable but now that I think back maybe I should have rested and then did Physical therapy. I did physical therapy and it went well. In the beginning I was just doing strenthening excersises(on my own) that were easy and I gradually used the machines specifically for leg strength or hip flexor. I went a good while without using the olyptical or treadmill. The physical therapist was trying to start using it to see if I would be ready for track meets soon becaus at that point i wasnt doing too much so it didnt hurt as much as before. The first time i used the olypictical it didnt bother me. I mean a little sharp pain but nothing too serious. On the treadmill the first couple times everything was good. The PT increased how much i was doing at Physical therapy bc it seemed like i was getting well.
THen my PT discussed how about i jump in a JV meet to see how it goes. Wellll...... it didnt go quite dandy. I jumped 4'8 and the jumps I were jumping werer terrible and very painful. During my indoor season I was jumping atleast 4'8'' no problem. Even the jumps at 4'8 were bad. After that meet I was in a lot of pain. And keep in mind the only reason I jumped at a JV meet was because I needed to see if i would be ready for the big meets because I had been out since Indoor State Meet. I went back to the PT the next day and we didnt know what happend everything seemed to be going fine. I had about 2 or 3 weeks till my league meet would happen and i really wanted to place high. We began doing less again in PT because he sorta just wanted me to get a good end to my freshman year of track. PT was going good the most I was doing was running A LITTLE and sometimes jumping on the machine meant for it. At the League Meet I did fabulous. I jumped 4'10 which isnt my best as of Eighth grade but i had been dealing with this injury so good enough. No pain at all with the help of this spray that makes it cold/numb called Kool and fit i believe. The PT then started running me more and using the jumping machine a lot more. There really wasnt taht much pain but there was some. I had went to the doctor before the league meet and he had givin me a Cortozone shot. It seemed to help as i was walking around.
At the District meet the pain was back and just nearly as bad as the JV meet. I jumped 4'8 AGAIN and even on 4'6 and 4'8 i nicked it. I go to the doctor after the district meet and we talk and he says this route we are taking isnt working. We are giong to try doing nothing for 6 weeks. He also says stretch at least 20 minutes a day. I tell my PT that is what the doctor said and he says Sierra i really dont think it is your flexiblility. I do take 6 weeks doing nothing. I go back to the doctor and he yells at me, I hadntbeen stretching enough. He says 6 more weeks doing nothing. During the 6 weeks I stretched at least 5 times a week for 20-30 minutes every night.
I then go see the other doctor that is in the office and we say this is getting tiring. We want some clue about what is going on. He then orders a MRI. He also says itcould be because of my back. (regardless no back MRI even though its over 5 months of this) I get the MRI done of the left and right hip and down. We get the results the next day "everything is fine, nothings wrongwith your hip" My doctor says we will try PT again. He says it is my Lower back and my hip hurting is just refered pain. I go into the same PT the next week and he says as far as i see it is not your back. I have no back pain what so ever. My PT seems to be concerned and think that my leg discrepancy could be the reason for all this. He says we will order a heal lift. I am VERY VERY annoyed with this injury especially because high jump is EVERYTHING to me and i have went to long :( what are your thoughts! Please and thanks. IF you read all this you are FABULOUS. You need to ask the Doc that you saw to refer to a Pediatric Orthopedic specialist and assess for whats called a SCFE, or a Slipped Capital Femoral Epiphysis. Its a rather common problem that often goes undetected even with advanced imaging as the presentation is VERY subtle. Someone who is used to looking at these can see them quite well, but for others it can easily go undetected. I can see maybe 30% of the ones I already have a diagnosis on!
Anyhow, the Pedi Ortho Dr would be a good route to at least explore and make sure they have you on some symptom management for the time being, other words something to manage whatever pain, swelling, inflammation you might be having. Limit stress on the hip for now and see what else you can find out. Prayers for your healing.
